Who we are

This website is operated by Reborn Health Partners LLC ("Reborn Health Partners", "we", "us"), a management services organization based in Massachusetts. The site is for practice owners and administrators who want to learn about our services. It is not a patient service and it is not a channel for medical care.

What this policy covers

This policy covers this website only. The services we provide to practices are governed by our written service agreements and, where patient information is involved, by a business associate agreement under HIPAA. Labs at Home is a separate company with its own website and its own privacy policy.

Information you give us

When you use the contact form, we receive what you type into it:

• your name, practice name and role
• your email address and, if you give it, your phone number
• the number of providers and the services you are interested in, if you tell us
• your message

Please don't include patient information in the form. It is not a secure channel for health information and we cannot act on clinical details sent through it. If patient information arrives anyway, we delete it.

Information collected automatically

Like any website, the servers that host this site record basic technical information when you visit — such as your IP address, browser type, the pages requested and the time of the request. Our hosting provider keeps these logs for security and operations.

This site does not use analytics or advertising cookies, and it does not load tracking scripts from third parties.

How we use it

• to reply to you and to work out whether we are a good fit for your practice
• to keep a record of business correspondence
• to keep the site secure and working

We do not sell your information, and we do not add you to a marketing list unless you ask us to.

Who we share it with

We share information only with the service providers that run this site for us — our website host and the email service that delivers contact form messages to us — and only so they can provide that service. We may also disclose information if the law requires it, or as part of a sale or reorganization of our business.

How long we keep it

We keep contact form messages for as long as we need them to respond to you and to keep a reasonable record of our business correspondence, and then we delete them.

Security

We use reasonable safeguards to protect the information you send us. No website or email system is perfectly secure, which is one more reason not to send patient information through this form.

Your choices

You can ask us what information we hold about you, ask us to correct it, or ask us to delete it. Email hello@rebornhealthpartners.com and we will respond.

Children

This site is for businesses and is not directed to children.

Changes to this policy

If we change this policy, we will post the new version on this page.
